For insurance purposes, we prefer that all new sleep patients have a physician or dentist referral before coming in to see us. 

If you are a referring providor, please contact our office to receive a complete referral packet. Additionally, if you would like to have one of our doctors do an in-service about sleep apnea, craniofacial pain, TMD or would like to visit our office yourself, please contact our main number.
